For All First-years, Transfers, First Year Student-Athletes, and Returning Student-Athletes
If you are an incoming first-year/transfer, first-year athlete for the 2025-26 school year, please wait until after May 15, 2025 to begin completing your pre-participation requirements using the ATS portal.
You must complete all items on your Admissions Portal relating to health services PRIOR TO completing any pre-participation forms related to Athletics. Failure to complete all health services forms will result in delayed clearance for sport participation.
All PPE documentation requirements are due to the athletic training staff by August 1. If not received by that date, we cannot guarantee your clearance for athletic participation in a timely manner or by your team's first day of practice. No exceptions.

General Steps
In order to be medically cleared, all student-athletes must complete the following general steps. Please click on the appropriate link listed below for more detailed instructions on the required PPE documentation.
1. Upon completion of the forms available, you will undergo a pre-participation physical examination under the direction of the LVC sports medicine team in order to be eligible for participation in intercollegiate sports or cheerleading.
2. Student-athletes will meet with LVC sports medicine staff with their respective teams at scheduled dates/times to complete ATR screening, receive their PPE examination with the team physicians, and will have all of their PPE documentation requirements reviewed at which time we can clear an athlete for play or direct them to finish any missing requirements.
3. Students will be administered the ImPACT concussion baseline test via the sports medicine staff. (Email to come later this summer)
4. All student-athletes who have had an injury or illness since your last playing season, that has required medical care in a facility outside of LVC,
must bring in all applicable medical documentation for our review. Please bring any copies of x-rays and/or MRI's on a CD, copies of doctor or specialist notes, return to play notes or restrictions, radiology reports, etc.
*** Athletes are not cleared until all the above steps have been met.
