If you are a returning student-athlete, please log in to your Lebanon Valley College Athletic Training electronic health record site at
lvc2.atsusers.com (computer) or
lvc4.atsusers.com (phone) to access your secure profile.
DO NOT create a new account. Log in with your original personal account. If you do not remember your login, please reach out to your designated athletic trainer and ask for your login information.
Please note: Use Google Chrome, Microsoft Edge, or Safari. The system is not compatible with Internet Explorer 10 or 11.
Once you have accessed your account, click on "Athlete Information" and ensure the accuracy of the following information.
Please fill out any NEW forms.
Review and update the information on each of the following tabs. When complete, click
verify at the bottom of the tab.
- General
- Insurance
- Contacts
- Upload front and back photos of Insurance cards to the Insurance tab in the ATS portal.
- Click on the insurance tab, click "add", complete the information needed in the yellow highlighted boxes, then upload photos of the front/back of your insurance card.
- NEW: Athlete forms - electronic signature required (do not print and mail, only submit electronically on ATS)
- Recertification Exam Form. This form is for sophomores, juniors, and seniors.
- You can access and fill out this form in the ATS portal under the "Forms" tab.
- Lebanon Valley College Intercollegiate Athletic Program Waiver
- Athletic Training Policies and Procedures Form
- Sudden Cardiac Arrest Educational Form

If you have had a significant injury or illness since your last playing season that required medical care, please send in or hand-carry to us as much information as possible. Most importantly, are copies of any radiology images, copies of the radiology report, and participation status or instructions for us from your healthcare provider. Your provider can send documentation via fax (717-867-6881), mail (see above), or secure email (yasinski@lvc.edu).
