First-Year/Transfers Pre-Participation Requirements

If you plan to participate in an intercollegiate sport or cheerleading, you will need to go to the Lebanon Valley College Athletic Training electronic health record site to create your secure online profile at lvc2.atsusers.com (computer) lvc4.atsusers.com (phone). Please note: Please use Google Chrome, Microsoft Edge, or Safari. Internet Explorer is not compatible. 
  1. Login with "new" as the username and password "new" to get started with a fresh account.
  2. Fill in the yellow boxes (at minimum) on all of the following tabs:  General, Insurance, Contact tabs. 
    1. Note that you must upload a front and back copy/picture of your insurance card to the portal in addition to the text to fulfill this requirement.
  3. Once you fill in the information on all 3 tabs, return to the General tab, scroll to the bottom, and click "save".
  4. You should see a prompt that indicates your account has saved, if not, you may have missed a section.
    1. Your account will not save in the system unless you have completed all information on the 3 tabs listed above.
  5. Once your portal is saved, you should see additional tabs at the top of your account, see further instructions below on the electronic signature forms that need to be completed.

Once you have created your personal account, write down and remember your athlete ID and password, click on "Athlete Information," and fill out the information under the following tabs:
  • Athlete forms - electronic signature required
            1. Lebanon Valley College Intercollegiate Athletic Program Waiver (do not print and mail, only submit electronically on ATS)

            2. Athletic Training Policies and Procedures Form (do not print and mail, only submit electronically on ATS)

            3. First-Year and Transfer Medical History (do not print and mail, only submit electronically on ATS)
  • Please make sure you dedicate at least 30-60 minutes to fill out this form. You CANNOT save your responses as you go. It WILL NOT save until all the questions are answered.
            4. Sudden Cardiac Arrest Education Form
 
  • Additional Paperwork
    • Sickle Cell Test Results
      • The NCAA requires all incoming student-athletes to submit a copy of their lab results of sickle cell trait status to participate in sports.
      • Each incoming student must either:
        1. Submit SCT test results from birth
        2. Request a test from their doctor
        3. Ask their assigned athletic trainer about getting a sickle cell test done at Penn State Health in Hershey, Pa. (The cost of the test will be added to your student account)
      • Sickle Cell Results may be submitted via mail or on ATS via the "Efiles" tab. Instructions on how to upload via the "Efiles" tab are listed in the "Get more detailed instructions" below.

Upon completion of these forms, you will undergo a pre-participation physical examination under the direction of the LVC Sports Medicine team in order to be eligible for participation in intercollegiate sports or cheerleading.

If you have had a significant injury or illness since your last playing season that required medical care, please send in or hand-carry to us as much information as possible. Most importantly, are copies of any radiology images, copies of the radiology report, and participation status or instructions for us from your healthcare provider.
