ANNUAL STUDENT-ATHLETE PRE-PARTICIPATION REQUIREMENTS
All required medical forms will become available to complete in your
ATS Athlete Portal by mid-May. You will receive an email from your Coach or Athletic Trainer making you aware when these forms are active.
Please refer below for instructions related to your status:
New Student-Athletes OR
Returning Student-Athletes.
To allow the Athletic Training Staff to review all information and ensure medical clearance in a timely manner, please have all requirements completed by August 1.
NEW LVC STUDENT-ATHLETES (FIRST-YEAR AND TRANSFER)
You must first complete all items in your
LVC Admissions Portal PRIOR TO completing any forms related to Athletics. Instructions for your Admissions requirements (due July 1) can be found in the welcome email you received from Admissions/Student Health Center. In addition, you can find guidelines on the
LVC Health Services webpage under both the
“Medical Records” and
“Student Responsibilities” tabs.
ATS Requirements (click HERE for detailed ATS instructions):
- Create ATS portal/account and complete the below required sections:
- General
- Insurance (requires legible front/back images of your insurance card)
- Contact
- Review, complete, and e-sign the following forms in your ATS account:
- LVC Athletics Program Waiver
- Athletic Training Policies & Procedures
- Sudden Cardiac Arrest Educational Form
- First-Year and Transfer Medical History (form may take 30-60 minutes to complete and WILL NOT save until all questions are answered)
Sickle Cell Trait Requirement:
- Please access the NCAA Sickle Cell Trait webpage and review the “Sickle Cell Trait Fact Sheet for Student-Athletes” on the right hand side of the page.
- The NCAA requires ALL new student-athletes to provide proof of a sickle cell solubility test. Documentation of your test results must be uploaded into your ATS portal under “E-files”. Options to obtain these results include:
- Contact your hospital of birth/pediatrician’s office and request a copy of your sickle cell trait status from your birth. Please be aware that this may take up to 6 weeks to process, so request this documentation ASAP so there are no delays in your eligibility being approved.
- Contact your local diagnostics lab and request sickle cell solubility test (i.e. Quest Diagnostics: sicklecelltesting.pwnhealth.com). Please be aware this may require a prescription from your physician. All fees will be your responsibility.
- If you live near a Penn State Health Medical Center facility, the LVC team physicians are able to provide you with a prescription to have your sickle cell solubility test completed. Please contact your athletic trainer for more information. All fees will be your responsibility.
Concussion Baseline Requirement (ImPACT):
- Please access the NCAA Concussion webpage and review the “ Concussion Fact Sheet for Student-Athletes” near the middle of the page.
- To complete your online baseline assessment, follow the instructions below to ensure accuracy and validity:
- Your computer (smartphones cannot be used) is plugged into an electrical outlet.
- Pop-up blockers are turned OFF.
- Test environment is quiet and distraction-free.
- You are well-rested and not under the influence of alcohol or drugs.
- Cell phones and other devices are turned off or away from the testing area.
- You’re sitting at a table or desk with a flat hard surface.
- All other browser tabs and programs are closed.
- An external mouse, a trackpad, or an iPad must be used throughout the exam.
- Enter Customer Code: C6FD8BBE37
- Click “Validate”, then “Launch Test”, and follow instructions to complete test.
- Once your test has been completed, the Athletic Training Staff will review your results. DO NOT email or upload your Confirmation Certificate.
- If your score is flagged as “invalid”, you must re-test and obtain a valid score. Click HERE to review possible reasons for receiving an invalid score.

Sports Physical Requirement:
- Your physical for Athletics (this is different from your required student physical for the Health Center; physicals completed by your pediatrician/primary care provider will not be accepted for Athletics clearance) will occur when you report to campus in August and be performed by the LVC Team Physicians from Penn State Health. You will receive an email from your Coach or Athletic Trainer with the specific date and time.
RETURNING LVC STUDENT-ATHLETES

- Login to your ATS Athlete Portal to review and, if necessary, update information in the following sections:
- General
- Insurance (requires legible front/back images of your insurance card)
- Contact
- Review, complete, and e-sign the following forms in your ATS account:
- LVC Athletics Program Waiver
- Athletic Training Policies & Procedures
- Sudden Cardiac Arrest Educational Form
- Returning Student-Athlete Annual Health History
- All returning student-athletes DO NOT need an annual physical examination.
- Juniors ONLY: You will be required to have your height/weight and blood pressure measured and recorded by the Athletic Training staff when you report to campus in August. You will receive an email from your Coach or Athletic Trainer with the specific date and time.
- Any returning student-athlete that has suffered an injury/illness that prevented you from competing since your last LVC playing season must upload in your ATS account a WRITTEN CLEARANCE from your treating physician to be cleared for athletic participation for the upcoming year.
