ATS Instructions

Dear Lebanon Valley College Student-Athletes:
 
The LVC Athletic Training Staff utilizes ATS as its electronic medical record for injury tracking and documentation. All student-athletes will use ATS to review, complete, submit, and/or electronically sign all required forms or documents needed for athletic clearance.  PAPER FORMS WILL NOT BE ACCEPTED.
 
First-year and transfer student-athletes must initially create an ATS account/portal before gaining access to any required forms.  If you are a transfer student-athlete whose prior institution also utilized ATS, you must create another account using “atslvc” as your new database.
 
If you are a returning LVC student-athlete and have forgotten your ATS username or password, please DO NOT create a new account.  Instead, access your ATS Athlete Portal and click the “Forgot your Password” or “Forgot your Username” link.
 
How to create your portal/account:
  1. Be prepared to complete all steps in one sitting or your portal will not save.
  2. Access the ATS Athlete Portal (bookmark this login link for later use).
  3. Type “new” for both username and password, then click the “Login” button.
  4. In the “General” tab, complete all yellow fields.
    • We recommend using your LVC email address (@lvc.edu).
    • If you are an incoming transfer student-athlete, please select the “YEAR” category that identifies you as a “Transfer”.  For example, Transfer Sophomore, Transfer Junior, etc. instead of Sophomore, Junior, etc. This will ensure you receive the appropriate forms to complete in your account.
  5. In the “Insurance” tab, complete all yellow fields.
    • To upload images at bottom of page, first select “Upload” tab.
    • Under the “Card Front” text, click the “Choose File” button and select a clear, legible image/pic of the front of your insurance card.
    • Under the “Card Back” text, click the “Choose File” button and select a clear, legible image/pic of the back of your insurance card.
    • Lastly, click the blue “Save” button at the bottom of the page (if “Save” button is not clicked, the images will not be uploaded).
  • Be sure insurance card images are clear and legible.
  1. In the “Contact” tab, complete all yellow fields.
  2. Once the above tabs have been completed, return to the “General” tab, scroll to the bottom, click “Save Athlete Information” and wait for the page to refresh.
  • A box will appear saying “Save Complete”.
    • Once your portal has SAVED you will then see several additional tabs at the top of your portal page: Paperwork, eFiles, Forms, etc.  These tabs will be used to complete your additional requirements (see below).
 
LVC Athletics Program Waiver
  1. Login to your ATS Athlete Portal and click the “Your Information” button followed by the “Forms” tab at the top of your portal page.
  2. On the left side of the page, click the drop-down arrow next to “Form Name” and select “LVC Athletics Program Waiver”.
  3. On the left side of the page, click the “New” button and wait for the waiver to load/pop-up below (can take up to 30 seconds; be patient).
  4. Review the contents of the waiver thoroughly, including any hyperlinks.
  5. Click the “Yes” bubbles throughout the document to signify that you have read and understand the material (See Figure 1 below).
  6. In the large text box, please type your name (See Figure 2 below).
  7. Click the checkbox next to “I agree to use electronic records and signatures” to move on (See Figure 2 below).
  8. Type your name in the signature box (See Figure 2 below) and click “Sign”.  Once the signature box refreshes, a blue text will appear at the top of the box with a timestamp (See Figure 3 below).
  9. Click the blue “Save” button and wait several seconds for the page to refresh - a green heading will appear that says “Save Complete” (See Figure 4 below).
 
Athletic Training Policies and Procedures Form
  1. Login to your ATS Athlete Portal and click the “Your Information” button followed by the “Forms” tab at the top of your portal page.
  2. On the left side of the page, click the drop-down arrow next to “Form Name” and select “Athletic Training Policies and Procedures”.
  3. On the left side of the page, click the “New” button and wait for the waiver to load/pop-up below (can take up to 30 seconds; be patient).
  4. Review the contents of the waiver thoroughly, including any hyperlinks.
  5. In the large text box, please type your name (See Figure 2 below).
  6. Click the checkbox next to “I agree to use electronic records and signatures” to move on (See Figure 2 below).
  7. Type your name in the signature box (See Figure 2 below) and click “Sign”.  Once the signature box refreshes, a blue text will appear in the top of box with a timestamp (See Figure 3 below).
  8. Click the blue “Save” button and wait several seconds for the page to refresh - a green heading will appear that says “Save Complete” (See Figure 4 below).
 
Sudden Cardiac Arrest Educational Form
  1. Login to your ATS Athlete Portal and click the “Your Information” button followed by the “Forms” tab at the top of your portal page.
  2. On the left side of the page, click the drop-down arrow next to “Form Name” and select “Sudden Cardiac Arrest Educational Form”.
  1. On the left side of the page, click the “New” button and wait for the Form to load/pop-up below (can take up to 30 seconds; be patient).
  2. Review the contents of the Form thoroughly.
  3. After completing the form, click the checkbox next to “I agree to use electronic records and signatures” to move on (See Figure 2 below).
  1. Type your name in the signature box (See Figure 2 below) and click “Sign”.  Once the signature box refreshes, a blue text will appear in the top of box with a timestamp (See Figure 3 below).
  2. Click the blue “Save” button and wait several seconds for the page to refresh - a green heading will appear that says “Save Complete” (See Figure 4 below).
 
First Year and Transfer Medical History (requires  30-60 minutes to complete)
  1. Login to your ATS Athlete Portal and click the “Your Information” button followed by the “Forms” tab at the top of your portal page.
  2. On the left side of the page, click on the drop-down arrow next to “Form Name” and select “First Year and Transfer Medical History”.
  3. On the left side of the page, click the “New” button and wait for the Form to load/pop-up below (may take up to 30 seconds; be patient).
  4. Read each question carefully and respond appropriately
    1. Each text box must be filled for the form to be saved.
    2. If you have nothing to type, enter “none” in the text box.
  1. There are 13 pages of questions, be sure to click the “Next” button after completing the last question on each page.
  2. After completing the form, click the checkbox next to “I agree to use electronic records and signatures” to move on (See Figure 2 below).
  3. Type your name in the signature box (See Figure 2 below) and click “Sign”.  Once the signature box refreshes, a blue text will appear in the top of box with a timestamp (See Figure 3 below).
  4. Click the blue “Save” button and wait several seconds for the page to refresh - a green heading will appear that says “Save Complete” (See Figure 4 below).
Returning Student-Athlete Annual Health History:
  1. Login to your ATS Athlete Portal and click the “Your Information” button followed by the “Forms” tab at the top of your portal page.
  2. On the left side of the page, click the drop-down arrow next to “Form Name” and select “Returning Student-Athlete Annual Health History”.
  3. On the left side of the page, click the “New” button and wait for the Form to load/pop-up below (may take up to 30 seconds; be patient).
  4. Read each question carefully and respond appropriately
    1. Each text box must be filled for the form to be saved.
    2. If you have nothing to type, enter “none” in the text box.
    3. There are 3 pages of questions, be sure to click the “Next” button after completing the last question on each page.
  5. After completing the form, click the checkbox next to “I agree to use electronic records and signatures” to move on (See Figure 2 below).
  6. Type your name in the signature box (See Figure 2 below) and click “Sign”.  Once the signature box refreshes, a blue text will appear in the top of box with a timestamp (See Figure 3 below).
  7. Click the blue “Save” button and wait several seconds for the page to refresh - a green heading will appear that says “Save Complete” (See Figure 4 below).
 
Sickle Cell Trait Documentation Upload
  1. Review the NCAA Sickle Cell Trait Fact Sheet for Student-Athletes.
  2. Login to your ATS Athlete Portal and click the “Your Information” button followed by the “eFiles” tab at the top of your portal page.
  3. Click the “Upload Document” button.
  4. In the “Description” box, please type “Sickle Cell Trait Results”.
  5. In the “Document Type” drop-down box, select “Sickle Cell”.
  6. Click the “Choose File” button and select a copy of your sickle cell test results.
  7. Click the blue “Upload” button to submit.
Documentation Upload (Doctor’s Clearance Note, Xray/MRI Results, etc.)
  1. Login to your ATS Athlete Portal and click the “Your Information” button followed by the “eFiles” tab at the top of your portal page.
  2. Click the “Upload Document” button.
  3. In the “Description” box, please type the name of the document or a description of the document (the “Document Type” drop-down box can be left blank; this is not required).
  4. Click the “Choose File” button and select the document you want to upload.
  5. Click the blue “Upload” button to submit.
